Use of Dexmedetomidine in a Patient Allergic to Clonidine Presenting for an Awake Craniotomy: a Case Report

The use of dexmedetomidine with concurrent scalp block is increasingly being utilized as an effective and safe anesthetic approach for awake craniotomy (AC). Dexmedetomidine alpha-2 adrenergic receptor (α2-AR) agonist dose-dependent sedative, analgesic, anxiolytic properties while preserving respiratory function. challenge the arises when patient has a clonidine allergy, which also α2-AR agonist. Currently, there no published literature regarding in allergic to clonidine. A 48-year-old male chronic obstructive pulmonary disease, sleep apnea, body mass index 54 allergy presents AC. Given goals surgery patient’s comorbidities, we planned monitored anesthesia care intravenous (IV) dexmedetomidine, remifentanil, propofol. We discussed potential risk reaction given his understood consented plan.
